SNAP Health Portal

Parents should have received an email saying “Reading Public Schools welcomes you to the Health Portal”. Please check junk email if not received.  Please go tohttps://www.studentehr.com/and create as login and password as directed.

If you are brought to a login and do not remember creating a password, please enter your email address and click on “forgot password” to reset.

You will then be brought to your child’s electronic health record, where you can update alerts,

medication orders and consent, and upload any documentation such as physicals and immunization records. You can also securely and directly message your child’s school nurse.

Please note you may add new contacts, such as grandparents or other caregivers, however any changes to primary caregiver information must be updated from the parent portal so that we have consistent information.

You will not be able to edit consent for transport and treatment in the event of an emergency, as that must be consented through the parent portal.

Please note that even if parents opt in to consent for administration of over-the-counter medications such as Tylenol and Ibuprofen, a doctor’s note will still be required for elementary students to receive any medication at school. Middle and high school students can receive Tylenol and Ibuprofen with parent consent.

To upload a doctor’s note, for instance a note with a new diagnosis or reason for absence: log into the portal, go to messages tab on the left side of the screen. You will be able to message the school nurse  and attach the file.
